What is a W-9?

W-9 is a tax form called Request for Taxpayer Identification Number and Certification.

It is used for:

  • Certifying that the vendor has provided the correct taxpayer identification number
  • Certifying that the vendor is not subject to backup withholding
  • Certifying that the vendor is a US citizen or a US resident alien, partnership, corporation, company, association, estate, or domestic trust permitted to operate in the United States

Who has to fill out a W-9?

All vendors paid by the City of South Bend must complete a W-9. The City of South Bend is required by the Internal Revenue Service to obtain these forms.
