TRA-SERVICE: Traffic Signal Complaint

Use only when a traffic signal is flashing, completely out or one or more lights in the signal aren't working.

DO NOT USE to report timing issues or other problems.

What is the issue with the signal?

Please be specific:

Is the signal flashing? What color?

Is the signal completely out or just one bulb? Which bulb?

If this is a multi-head intersection, which signal head is having an issue?  #1 signal head is the one closest to the center, #2 signal head is the one closest to the curb.

Where is the signal located?

Which intersection is having the issue?  (Ex: Lafayette and South)

Which direction are you traveling? (Ex: northbound on Lafayette)

What time of day did you experience the problem?

When will the issue be addressed?

The first order of business will be to make the intersection safe.

Traffic & Lighting personnel will evaluate the issue and effect repairs as soon as possible.
